When this procedure is needed

How the visit goes
- 1Diagnostics
A panoramic X-ray, a cephalogram, a photo record and a digital scan of both jaws. From these the gnathologist sees not only where the teeth stand but how the jaws sit relative to each other.
- 2Plan and visualisation
The orthodontist shows the calculated tooth movement and the timeline. At this point it becomes clear how many trays will be needed and what the treatment will cost.
- 3Wearing the appliance
Trays are changed every one or two weeks; fixed appliances are adjusted every month to six weeks. The review visits are short.
- 4Retention
Once the appliance comes off, a retainer goes on: a fixed wire on the inner surface or a night tray. Without it the teeth move back — this is not a formality.
After the procedure
- Aligners are worn 20–22 hours a day. Less than that and the timeline stretches, and the plan stops matching reality.
- Take the aligners out to eat and for anything but water: hot drinks deform them, coloured ones stain them.
- Rinse the aligners in cool water and clean them with a brush and no paste — paste scratches the plastic and clouds it.
- A retainer is worn for years after treatment, and a fixed one stays permanently. That is what keeps the result.
- A review visit every six to eight weeks, even when everything is going well.

- How long does treatment take?
- From six months for a small correction to two years for a complex bite. The orthodontist gives an exact figure after diagnostics, not by eye.
- Aligners or fixed appliances — which to choose?
- Aligners are invisible and come out for meals, but they need discipline: 20–22 hours a day. Aesthetic fixed appliances work without the patient's involvement and handle harder cases.
- Will the teeth move back?
- Without a retainer, yes — and that is not a scare story. A retainer is a required part of the treatment, not an optional extra.
- Is there an age limit for aesthetic fixed appliances?
- There is no upper limit. Teeth move through bone at any age — at forty or sixty just as at fifteen, only more slowly. For an adult one thing matters: healthy gums and any caries treated before starting. With periodontitis treatment is still possible, but the plan is different and more cautious.
- Do aesthetic fixed appliances ruin the enamel?
- The bracket itself does not. Plaque collecting around it does: clean carelessly and a year later white spots are left where the brackets were, which is early caries. So fixed appliances call for an interdental brush, a water flosser and a hygienist visit every three or four months. Aligners make this easier: they come out before eating and brushing.
